Admission Forms

Grades 1 – 8

New Students:  New students registering for Grades 1-8 at St. Joseph Regional Catholic School must submit the following forms prior to the start of the new school year:

• SJRCS Admission Form
• Parent Statement Form
• Immunization Record (filled out by applicant’s primary care physician)
• Copy of Birth Certificate

The following need only be submitted if applicable:

• Copy of Baptismal Certificate
• SJRCS Supported Rate Form
• Extended Care Program Information and Costs
• Extended Care Program Registration Form

Current Students:  All SJRCS students re-registering for the next school year must submit the following forms by the date determined by SJRCS:

• SJRCS Re-Registration Form (provided by SJRCS in January)
• Immunization Record (completed by student’s primary care physician)

The following need only be submitted for re-registration if applicable:

• SJRCS Supported Rate Form (only if a change in status is requested)
• Extended Care Program Information and Costs
• Extended Care Program Registration Form

 

Kindergarten

All students registering for the Kindergarten program must complete and submit the following forms to the Kindergarten Director prior to the start of the new school year, or prior to the child’s first day if entering the program within a school year:

• St. Josephs Kindergarten Admission Form
• SJRCS Admission Form
• Immunization Record (filled out by applicant’s primary care physician)
• Copy of Birth Certificate

The following need only be submitted if applicable:

• Extended Care Program Information and Costs
• Extended Care Program Registration Form

 

Preschool

All children registering for the Preschool program must complete and submit the following forms to the Preschool Director prior to the start of the new school year, or prior to the child’s first day if entering the program within a school year:

• St. Joseph Preschool Admission Form
• Immunization Record (filled out by applicant’s primary care physician)

The following need only be submitted if applicable:

• Extended Care Program Information and Costs
• Extended Care Program Registration Form
