Admission Forms

Grades 1 – 8

New Students:  New students registering for Grades 1-8 at St. Joseph Regional Catholic School must submit the following forms prior to the start of the new school year:

The following need only be submitted if applicable:

Current Students:  All SJRCS students re-registering for the next school year must submit the following forms by the date determined by SJRCS:

  • SJRCS Re-Registration Form (provided by SJRCS in January)
  • Immunization Record (completed by student’s primary care physician)

The following need only be submitted for re-registration if applicable:

 

Kindergarten

All students registering for the Kindergarten program must complete and submit the following forms to the Kindergarten Director prior to the start of the new school year, or prior to the child’s first day if entering the program within a school year:

Preschool

All children registering for the Preschool program must complete and submit the following forms to the Preschool Director prior to the start of the new school year, or prior to the child’s first day if entering the program within a school year:
